Post induction of anaesthesia, your patient’s BP falls to 68/40. The patient’s heart rate is 70/min. There is no improvement despite 500mcg of phenylepherine (100mcg boluses at a time) and 500ml ringers lactate fluid bolus. You perform a FoCUS study. The patient’s BP is low post induction of anaesthesia. Common causes include LV systolic failure, hypovolaemia and decreased SVR. The FoCUS study shows severe LV systolic dysfunction. Additionally, the RV is dilated and has reduced systolic function. There is no pericardial effusion present. The aortic valve is normal. The mitral valve is not seen clearly to make an assessment. The subcostal 4C and IVC were not obtained in this study.
Conclusion: Hypotension secondary to LV systolic failure
